Introduction
Equipment failure and its attendant cost are extremely important in today's petroleum industry. To optimize operations by a failure -control program ,basic information is required define the nature and magnitude of problems and to estimate the economic stakes.
System for obtaining this information have the following goals in common:
(1) To determine the cause of equipment failure.
(2) To help set specifications for equipment .
(3) To predict future performance of equipment.
(4) To follow the economic impact of remedial actions.
Failure documentation
Failure control in its simplest form is failure analysis with the goal of achieving corrective action. Premature equipment failures are usually a result of one or more of the following:
*Design deficiencies
*Improper selection of material (material deficiencies)
*Error in assembly.
*Service conditions not considering in design.
As the first step of the damaged component must be secured. Parallel to this , information should be collected. The following points should be borne in mind if the failure is examined on site:
Macroscopic examination of the original state of the failure(Visual inspection) Photographs should be taken if possible (original state) Recording of the general impression and surrounding circumstances.
