TOFD as per code case 2235

For qualifying a PDR using TOFD as per CC 2235for pressure vessels please clarify my doubts regarding the following

While testing a 80 mm butt weld I was able to get a toe crack ( weld induced ) but was not able to detect a notch 6mm deep , 50mm long , 5mm wide at the toe of the weld.
Two depth zones were used to test the whole 80mm weld.The set up used is depth zone 1-70 degree 5 Mhz 6mm dia with a PCS of 137mm focussed at 25mm 2T. Depth zone 2-60 degree 5Mhz 6mm dia with PCS of 233mm focussed at 5/6 T.
