code case 2235-2: qualification block

I would like to better understand the a.m. code case requirements for the qualification block.

The code case says:

"The block shall include al least two planar defects ..... ... no larger than the flaw in Table.....

1)What is the larger meaning? Does it refers to the defect height or length or both?

2) May I use a machined notch as a planar surface flaw?
3) How could I make a planar subsurface flaw?
