Dear Sir,
My reply is based on memory only and without proof.
I remember 1968 ASME VIII/1 Code as a gray coloured cover page bound book. And for NDT requirements no separate book was required to be referred. But 1971 there was separate volume as Section V.
For your further information, in one inquiry to ASME, I referred to one clause of 1977 edition of Section V and the answer was ASME does not have 1977 edition to refer.
Based on above two –
1. ASME V did not exist from the beginning.
2. It will be difficult to trace the true history. Most probably the first edition of ASME V was published in 1971.

I can so understand why ASME up to now is silent.
In my article (history of NDT) I assume 1971 as most probable starting of ASME V.
