looks acceptable to me as well with out cross checking wall thickness and accept criteria.

not darker than parent material either i guess

looks like local negative reinforcement at the weld root, possable stress riser due to sharp appearance, definite failure point for this weld under extreme stress.

What is this pipeline for, what is flowing through it ?

Also which specification are you working to ?

As this will determine whether or not it is acceptable, as we know the size and wt.

look suspiciously like a burn through given that the profile of the root bead changes from thin to wide either side of the discontinuity.
I would like to know the welding technique used i.e. stove pipe. The cap weave looks typical of a stove technique. What was the o'clock position?

Jointly designed by PH Tool and Holloway NDT, the blocks feature side-drilled hole reflectors positioned within novel compound-curves, each with multiple thickness steps.
