What is the purpose of the 5 small holes that are grouped near the bottom of the IOW block?

The IOW beam profile block is used to plot the vertical and horizontal beam profiles of a transducer.

According to The Welding Institute's publication "Procedures and recommendations for the ultrasonic testing of butt welds" the five holes that you describe are "an alternative site for a second set of holes (spaced to suit the user) to meet cases where it is desired to test the resolving power of large low-frequency probes e.g. initial search probes, at angles greater than 45 degrees".
